Mourinho was ordered off the technical area by the referee Anthony Taylor after protesting about apparent time-wasting and sat with ‘Blues’s’ supporters in the stand.
The 50-year-old Portuguese has until Thursday, October 24 to respond to the charges.
Mourinho spent the last 20 minutes of the controversy-plagued match sitting among supporters behind the Chelsea dugout after his dismissal.

Chelsea striker Samuel Eto’O was the brain behind the other controversy which took place in the match. The Cameroonian kicked the ball away as Cardiff goalkeeper David Marshall bounced it.
According to FIFA rules, Eto’O’s actions should have been considered a foul, but referee Taylor incorrectly allowed play to continue.
The last time Mourinho was ordered to the stands was in May, while he was still in charge at the Bernebeu. He protested after a decision went against his team in the Copa del Rey final defeat by city rivals Atletico.
